Rajiv Nagar, At/P.O. Aiginia, Bhubaneswar, Odisha-751019 declare that, I have changed my name

from "Ratanmani Dhir" to "Kusum Kumari Dhir''. Henceforth, I shall be known as Kusum Kumari

Dhir for all purposes.
