By virtue of an affidavit sworn before the Executive Magistrate, Narasinghpur on the 25th

January 2024, I, Champa Mahalik, W/o Rabindra Mahalik, Vill. Lembo, P.O. Alara, P.S. Narasinghpur,

Dist. Cuttack, State Odisha declare that, I have changed my name from "Somabari Mahalik " to

"Champa Mahalik''. Henceforth, I shall be known as Champa Mahalik for all purposes.
