2024, I, Himanshu Sekhar Mallik, son of Surendra Kumar Mallik, Village Haribhaktapur, P.O. Lalitagiri,

P.S. Mahanga, Dist. Cuttack, State Odisha declare that, I have changed my name from "Hiamnshu

Sekhar Mallik " to "Himanshu Sekhar Mallik''. Henceforth, I shall be known as Himanshu Sekhar

Mallik for all purposes.
