By virtue of an affidavit sworn before the Notary Public, Bhubaneswar on the 16th February

2024, I, Laxmikanta Dalai, aged 38 years, S/o Shri Mansingh Dalai, resident of Senapatia,

Chandanpur, Malatipatpur, P.S. Chandanpur, Dist. Puri, have changed my name from "Laxmikanta

Dalai" to "Laxmikanta Mahapatra''. Henceforth, I shall be known as Laxmikanta Mahapatra for all

purposes.
