P.S. Bolgarh, Dist. Khurda, Odisha. Declare in Presence of Notary Public Sworn this Affidavit.

I have change my name from Champa Mani Sahoo to Champa Mani Dei. Henceforth, I shall be

known as Champa Mani Dei for all purpose.
