8th November 2023, I, Laxman Dhanwar, son of Basudev Dhanwar, Vill. Patrapali, P.O. Ajikela,

P.S. Bisra, Dist. Sundergarh, Odisha, declare that, I have changed my name from "Lauda Dhanwar ''

to "Laxman Dhanwar''. Henceforth, I shall be known as Laxman Dhanwar for all purposes.
