Dist. Bargarh, PIN-768027, Odisha declare that, I have changed my daughter's name from

"Riddhi Biswal'' to "Riyasha Biswal''. Henceforth, my daughter Riddhi Biswal shall be known as

Riyasha Biswal for all purposes.
