December 2023, I, Mohinee Swain, W/o Dusmanta Swain, At Rathijema Charimania, P.O. Balakati,

P.S. Balianta, Dist. Khordha, Odisha-752100 declare that, I have changed my name and surname

from "Mohini Sahoo'' to "Mohinee Swain''. Henceforth, I shall be known as Mohinee Swain for all

purposes.
