By virtue of an affidavit sworn before the Notary Public, Bhubaneswar on the 12th January

2024, I, Madhumita Harichandan, W/o Late Bhagirathi Harichandan, Village Baghamunda,

P.O. Nuagaon, Badabhuyin, P.S. Brahmagiri, Dist. Puri, Odisha, declare that, I have changed my

surname from "Madhumita Nayak'' to "Madhumita Harichandan''. Henceforth, I shall be known as

Madhumita Harichandan for all purposes.
