By virtue of an affidavit sworn before the Executive Magistrate, Cuttack vide Serial Number

667, dated the 1st February 2023, I, Chetna Priyadarshini, D/o Purna Chandra Mohapatra, resident

of At Bangalisahi, Basanti Lane, P.O. Telengabazar, P.S. Purighat, Dist. Cuttack, Odisha-753009,

have changed my name from "Prarthana Mohapatra'' to "Chetna Priyadarshini''. Henceforth, I shall

be known as Chetna Priyadarshini for all purposes.
