By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar on the 9th

February 2024, I, Anirudha Swain, S/o Late Kumar Swain, Vill. Rathijema Charimania,

P.O. Balakati, P.S. Balianta, Dist. Khordha, Odisha, declare that, I have changed my name from

"Babaji Swain'' to "Anirudha Swain''. Henceforth, I shall be known as Anirudha Swain for all

purposes.
