By virtue of an affidavit sworn before the Executive Magistrate, Ranpur on the 12th February

2024, I, Santosh Trinath Pradhan, S/o Trinath Pradhan, At Nuapada, P.O. Kerandatangi,

P.S. Chandpur, Dist. Nayagarh, PIN-752024, Odisha, declare that, I have changed my name from

"Babrubahan Pradhan'' to "Santosh Trinath Pradhan''. Henceforth, I shall be known as Santosh

Trinath Pradhan for all purposes.
