2024, I, Indramani Parida, S/o Late Satyabadi Parida, At/P.O. Pahanga, Via Niali, P.S. Niali,

Dist. Cuttack, Odisha declare that, I have changed my name from "Mohan'' to "Indramani Parida''.

Henceforth, I shall be known as Indramani Parida for all purposes.
