By virtue of an affidavit sworn before the Notary Public, Cuttack on the 27th February 2024,

I, Keshab Chandra Senapaty, S/o Late Khatu Senapaty, Permanent resident of Village Balikuda,

P.O. Saudia, P.S. Balichandrapur, Dist. Jajpur-754292, have changed my name/surname from

"K.C. Senapaty'' to "Keshab Chandra Senapaty''. Henceforth, I shall be known as Keshab Chandra

Senapaty for all purposes.
