dated the 1st July 2024, I, Smt. Jyostna Sahu, W/o Late Bichitrananda Sahu, At Bhagaban Pur,

P.O. Nuabag, P.S. Hindol, Dist. Dhenkanal, declare that, I have changed my name from "Jyotsna

Sahoo'' to "Jyostna Sahu''. Henceforth, I shall be known as Jyostna Sahu for all purposes.
