By virtue of an affidavit sworn before the Notary Public, Cuttack on the 13th August 2024,

I, Sarbeswar Behera, S/o Late Mukunda Behera, permanent resident of Vill./P.O. Kusupangi,

P.S. Banki, Dist. Cuttack, Odisha-754007, have changed my son's name from "Gagan Behera'' to

"Pruthwiraj Behera''. Henceforth, he shall be known as Pruthwiraj Behera for all purposes.
