I, Minakshi Mallick, D/o Bhanja Mallick, permanent resident of At Shrambukia, P.O. Rutungia,

P.S. Baliguda, Dist. Kandhamala, Odisha have changed my name from "Hanami Mallick'' to "Minakshi

Mallick''. Henceforth, I shall be known as Minakshi Mallick for all purposes.
