By virtue of an affidavit sworn before the Executive Magistrate, Cuttack on the 21st May

2024, I, Anil Gupta, S/o Sibnath Prasad Shaw, At Dula Dei Road, Andarpur, P.O. Kalyaninagar, P.S.

Madhupatana, Town/Dist. Cuttack have changed my surname from "Shaw'' to "Gupta''. Henceforth,

I shall be known as Anil Gupta for all purposes.
