P.O. O.M.P. Line, P.S./Dist. Jharsuguda, Odisha-768204 declare that, I have changed my surname

from "Dambar Bahadur Alley" to "Dambar Bahadur Ale''. Henceforth, I shall be known as Dambar

Bahadur Ale for all purposes.
