P.O. O.M.P. Line, P.S./Dist. Jharsuguda, Odisha-768204 declare that, I have changed my name

from "Bhagi Ram Gurung" to "Santa Bahadur Gurung''. Henceforth, I shall be known as Santa

Bahadur Gurung for all purposes.
