By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar on the

16th May 2025, I, Yahan Rout, Address S-10, Pravat Villa, Kalarahanga, Patia, near Rotary

Eye Hospital, P.O. KIIT, Dist. Khorda, Odisha, PIN- 751024 declare that I have changed my

name from "Yahana Rout'' to "Yahan Rout'' and gender from female to male. Henceforth,

I shall be known as Yahan Rout and my gender shall be male for all purposes.
