I, Ranganadane, son of Rattinavelou, Hindu, aged about

54 years, residing at No. C120, V. P. Singh Nagar,

Caliditalcoupom, Puducherry, do hereby solemnly affirm

and state on oath as follows:

I state that I was born on 7-6-1957 at Caliditalcoupom

and my birth was registered under Registration No.196

of the Birth Records of Mannadipet Commune Panchayat,

Thirubhuvanai and as per my Birth Certificate, my name

is ‘Ranganadane’ whereas in other public records such

as Family Ration Card, Voter’s Identity Card, my name

religion by accepting Buddha Dhamma Diksha

at Holy Dikshabhumi, Nagpur organised by Param Pujya

Dr. Babasaheb Ambedkar Smarak Committee, on my

own accord willingly without any compulsion,

seduction, coercion or by force by any external force.

The Param Pujiya Dr. Babasaheb Ambedkar Smarak

Committee, Dikshabhumi, Nagpur-440 010 (Maharashtra)

has issued Certificate of Buddha Diksha, dated

14-10-2010 to this effect.

That from the date of 14th October 2010 onwards,

I have been maintaining and following ‘Buddhist’

religion and I would henceforth follow, observe and

maintain Buddhist religion only.
