In conformity with rule 8 of the Pondicherry Municipalities (Grant of Licences and Permissions)

Rules, 1976, promulgated in this Union Territory by the Pondicherry Municipalities Act, 1973, objections, if

any, to establish and the use of the power of the above industries are invited to reach the undersigned within ten

days from the date of publication of this notice in the official gazette.

The objections so received within the specified time will be considered by the Council,

Yanam Municipality, Yanam.

CHANGE OF NAME

I , Thamizhselvi, wife of Janarthanan, Hindu,

aged 31 years, and residing at the house bearing

door No. 23, Reddiar Street, Thimmanayakkanpalayam,

Puducherry-605 007, do hereby solemnly and

sincerely affirm and state as follows:

I am the deponent herein. I am well aware of the

facts of my affidavit. I swear this affidavit to state

that the names ‘Thamizhselvi’ and ‘Kalaiselvi’ refer to,

identify and relate to one and the same person.

I state that my registered name is ‘Thamizhselvi’

the said statement of mine is substantiated through

my Certificate of Birth, issued by the Registrar

of Births and Deaths, Puducherry Municipality,

registered with it under No. M/1979/00981 of the

year 1979.

I further state that though my registered name is

‘Thamizhselvi’ my call name is ‘Kalaiselvi’ as such

in the Family Ration Card under Reference No. 345819,

issued by the Department of Civil Supplies and

Consumer Affairs, Puducherry, my said call name has

been

mentioned

as

‘Kalaiselvi’

instead

of

‘Thamizhselvi’.

Therefore I do hereby declare that the said name

‘Thamizhselvi’ as found in my said Certificate of

Birth, and the name ‘Kalaiselvi’ as found in my

Family Ration Card, refer to, identify and relate to

one and the same person that is myself the

deponent herein.

I state that what are all stated in the above

paragraphs are true to the best of my knowledge,

belief and information.
