By this deed, I, the undersigned Kaliaperumal (new
name) formerly called Balakrishnan, son of Gopalsamy
alias Narayanasamy, Hindu, aged about 60 years and
residing at No. 211, Kamaraj Salai, Pillaithottam,
Puducherry-605 013, do hereby solemnly affirm and state
on oath as follows:
That I have been so far called as ‘Balakrishnan’
and the same has been entered in my Family Ration
Card. That I have now changed my name as
‘Kaliaperumal’ for numerological and sentimental
reasons and I shall hereafter be known by this name.
I hereby declare that I shall at all times, hereafter
in all records, deeds and writings and in all
