I, Counasoundary, wife of Ezhumalai, Hindu,

aged about 39 years, and residing at No. 40, North Street,

Pillaichavady, Puducherry, solemnly affirm as under:

That I am the deponent herein.

I state that my name is ‘Counasoundary’ as found

in my Birth Certificate and Record Sheet. I am also

known and called as ‘Vasuki’. I further state that

I got married with one Ezhumalai on 5-2-1990

such in my GPF account under PRESS 286 my name

has been mentioned as ‘Pushpa, A.’ while being so

in the Office Memorandum, dated 1-10-2010 and the

Office Order, dated 18-10-2010, under Serial No. 11 my

name has been mentioned as ‘A. Pouchepa’, instead

of ‘Pushpa, A.’.

Therefore, I do hereby state that the name

‘Poushpa’ as found in my said Certificate of Birth and

the name ‘Pushpa, A.’ as found in my GPF account

under PRESS 286 and the name ‘A. Pouchepa’

as found in the Office Memorandum, dated 1-10-2010

and the Office Order, dated 18-10-2010, under Serial

No. 11 and the name ‘G. Pushpa’ as found in the said

Gazette notification refer to, identify and relate to one

and the same person, that is myself the deponent

herein. But henceforth I would be known and

identified by the name ‘G. Pushpa’ alone. Hence

I request this authority may be pleased to pass an

order to replace my name in service book and official
