I am the deponent herein. I did M.Com., M.A.,

Assistant Educational Officer during my service from

and service records contain my original name. During

to astrological sentiments, I want to resume to my

Pondicherry, my name and date of birth were mentioned

as ‘N. Ezhilan’ born on 28-3-1969. Similarly in my Voter’s

Identity Card, bearing No. 042199, my name is entered as

‘Ezhilan’’. In my Family Ration Card, bearing No. 340774,

my name is entered as ‘Ejilan’. In my Caste Certificate,

bearing C. No. 1265/TOP/C/04, issued by Taluk Office,

Government of Pondicherry, my name is correctly entered

as ‘Ejilan’. Hence my correct name is only ‘N. Ejilan’ born

on 28-3-1969 at Pondicherry and all my names stated in

all the above said documents belong and refer to me

only and as such I furnish this affidavit towards my

Identity Proof that both my names ‘Ejilan’ and ‘Ezhilan’

belong and refer to me only and I also furnish this

affidavit towards my Identity Proof in respect of

my above said corrected School Certificate that my correct

name is only ‘Ejilan’ and correct date of birth is

28-3-1969.
