I, Mona Leena alias Mohana, wife of (late) Murugan,
Hindu, aged about 33 years and residing at No. 17,

I state that I am the deponent herein.
I state and declare that I was born on 20-7-1977
at Pondicherry and the same was duly registered before

same person i.e., myself i.e., ‘Mona Leena alias Mohana’.
I have not concealed any real facts.
