by the Department of Civil Supplies and Consumer
Affairs, Puducherry and in the Certificate of Birth
of my daughter namely Durga alias Jayasri, in the
Certificate of Birth of my son namely Karthikeyan
my name has been mentioned as ‘Kokila’ instead
of Coquily.
Therefore, I do hereby declare and state that
the said name ‘ Coquily’ as found in my Certificate
of Birth and the said name ‘Kokila’ as found in my
said Family Ration Card, daughter’s and son’s
Certificate of Births, refers to, relates to, identifies
one and the same person that is myself the
deponent herein. I shall henceforth be known and
identified by the name ‘Coquily’ only.
Solemnly and sincerely affirmed and signed before
