‘T.S. Charavanan’. This changed name ‘T.S. Charavanan’
is mentioned in my Identity Card issued by my
Department and the Pass Book issued by UCO Bank,
Puducherry. The above initial ‘T’ refers my paternal
grandfather’s name and the initial ‘S’ refers my
father’s name. Therefore, hereinafter I will be known,
called and identified by my assumed name only as
‘T.S. Charavanan’. I affirm that I hereby relinquish
