said statement of mine is substantiated through my

Certificate of Birth, issued by the Registrar of Births and

Deaths, Ariankuppam Commune Panchayat, Puducherry.

I further state that though my registered name is

‘Valarmathi’, my call name is ‘Pallavi’. As such in my

Family Ration Card under No. 31500, issued by the

Department of Civil Supplies and Consumer Affairs,

Puducherry my name has been mentioned as ‘Pallavi’

instead of ‘Valarmathi’.

Therefore, I do hereby declare and state that the said

name ‘Valarmathi’ as found in my Certificate of Birth

and the name ‘Pallavi’ as found in my said Family Ration

Card refer to, relate to and indentify one and the same

person that is myself the deponent herein. Henceforth,

I shall be known and identified by the name ‘Valarmathi’

only.
