CHANGE OF NAME

I, Mandjalandy, son of Couppoussamy, Hindu, aged

about 69 years, residing at No. 10, Kennedy Street,

Kamaraj Nagar, Puducherry, do hereby solemnly affirm

and state on oath as follows:

I am the deponent herein and swearing this

affidavit on my behalf.

That, I was born to my parents Couppoussamy

and Mouniammalle on 16-1-1943 at Puducherry

and the same was registered before the Puducherry

Municipality under Registration No. P/1943/00121

on 21-1-1943. In this Birth Certificate my name was

given as ‘Mandjalandy’.

I do affirm and state that the name found in my

Election Identity Card, as ‘Manjalandy’ issued by the

Election Commission of India.

I do affirm and state that the name found in my

Family Ration Card as ‘Manjananthi’ and ‘Manjanandi’

are refer to one and the same person.

I do affirm and state that the abovesaid facts are

true to the best of my knowledge, belief and

information and nothing material facts has been

‘Durai’ found in my Voter’s Identity Card, issued by

No. PY/01/002/003431, refer to one and the same

person i.e. myself. I do affirm that I shall hereafter

Chandan Birth Certificates my name was spelled

and entered as ‘Jayakumar’. In my Voter’s Identity

Card No. PY/01/021/006677, my name was spelled as

‘Jayakumar’ and the name ‘Jayakumar Counder’ found

in my Family Ration Card issued by the Department

of Civil Supplies and Consumer Affairs, Puducherry

bearing Card No. 020818.

I do hereby declare that the abovesaid names

‘Djeacoumar’, ‘Jayakumar’ and ‘Jayakumar Counder’

refer to one and the very same person i.e., myself only.

correct to the best of my knowledge and belief and

of proof of identity of my father’s names.
