I, D. Sandhya, born on 16-7-1972 and residing at

Tagore Nagar, Lawspet, Puducherry-605 008, do hereby

solemnly and sincerely affirm and state on oath

The name ‘A. Deela Priskila’ found in my

Transfer Certificate S. No. 3474 and the name

‘Sandhya’ found in my PAN Card No. AAUPS7225B

that is myself the deponent herein. I shall

henceforth be known and identified by the

name ‘D. Sandhya’, only.

CHANGE OF NAME

I, Preethi Parimelazhagan, daughter of Parimelazhagan,

Hindu, aged about 26 years, residing at No. 4, Mariamman

Koil Street, Velanthottam, Thilaspet, Puducherry-9, do

hereby solemnly affirm and sincerely state as follows:

I state that I was born to my parents Parimelazhagan

and Mageswari on 17-8-1986 at Government Hospital,

Tindivanam. My name is ‘Preethi’, but in all my records

my name has been mentioned along with my father’s

name, namely Parimelazhagan. My name ‘Preethi’ has

been mentioned only in my Family Ration Card.

I further state that I married my husband Magesh

on 19-3-2012.

I further state that by virtue of my marriage, I hereafter

be called as ‘Preethi Magesh’ (or) ‘M. Preethi’.

Hereafter, I may be called and used as ‘Preethi Magesh’

(or) ‘M. Preethi’ for all purposes instead of ‘Preethi

Parimelazhagan’.

The names ‘Preethi Magesh’ and ‘M. Preethi’ are

referring one and the very same peson i.e. myself only

Hence this affidavit.

CORRIGENDUM

Holidays for all Banking Institutions including

Co-operative Banks in Union Territory of Puducherry

for half yearly closing of Bank Accounts i.e., Monday,

the 30th September, 2013, which was notified

vide G.O. Ms. No. 56, dated 14-11-2012 of Home

Department, Puducherry is deleted.
