found in my Livret De Famille Book, issued by the

Etat Civil Officer, Puducherry Municipality, the name

‘Cadirvelou’ which is found in my Voters’ Identity

Card issued by the Election Commission of India,

bearing Card No. PY/01/006/039023, the name

‘Cadirvelou’ found in my Aadhaar Card, issued by

the Unique Identification Authority of India, bearing

No. 6325 3682 0343, and the name ‘Kathirvelu’

(English Spelling) which is found in my Secondary

School Cumulative Record, issued by the Principal,

Government Higher Secondary School, Madagadipet,

Puducherry. That I declare that both the names

‘Cadirvelou’ and ‘Kathirvelu’ are one and the same

referring my father.

That I hereby declare that I shall at all times

hereafter in all records, deeds and writings and in all

proceedings, dealings and transactions, private as

well as upon all occasions whatsoever use and sign

the name of ‘C. Radjassegarane’ as my name in place

and in substitution of my former name.

The above deposition is true and correct to the

best of my knowledge and belief.
