Certificate, dated 3-6-2000, issued by Central Board of
Secondary Education, Mysore and the name ‘Krishna
Kumar Chandran’ as found in the Mark Statement,
dated 23-5-2002, issued by the Central Board of
Secondary Education, Chennai and also in the Passport
No. F 6001742, dated 9-1-2006 and valid till 8-1-2016,
issued by Regional Passport Officer, Bangalore refer
to one and the same person viz., my son.
I do hereby declare and state that my son has
changed his name as ‘Krishna Kumar Chandran’
abandoning and relinquishing once for all his earlier
names ‘Mithunchandra’ and ‘Krishna Kumar. C’. He
would henceforth be known and called as ‘Krishna
Kumar Chandran’ for all purposes.
I do hereby declare that what are all stated above
are true to the best of my knowledge, belief and
information. I have not concealed any facts.
