I , t h e u n d e r s i g n e d , S a v i t h a , d a u g h t e r o f

Subramaniam, aged 36 years, residing at No. 5, Third

Cross Street, Calve Bungalow, Puducherry-605 001, do

hereby solemnly affirm and state on oath as follows:

1. That my daughter Janani, born on 4-12-2007,

hereby wholly renounce/relinquishes and abandons

the use of her former name, ‘M. Janani’ and in place

thereof, assumes from this date the name,

‘S.L. Janani’ and so that she may hereafter be

called, known and distinguished not by her former

name, but by her assumed name, ‘S.L. Janani’.

2. That for the purpose of evidencing such

determination, I declare that she shall at all times

hereafter in all records, deeds and writings and in

all proceedings, dealings and transactions, private

as well as upon all occasions whatsoever use the

name of ‘S.L. Janani’ as her name in place and in

substitution of her former name.

3. That I expressly authorise and request all

persons in general, relatives and friends in

particular, at all times hereafter to designate and

address my daughter by her such assumed name,

‘S.L. Janani’.

I am the deponent in this affidavit and well-

acquainted with the contents of the same.

I was born on 13-2-1964 at Karayambuthur Village,

Bahour Commune, Puducherry to my parents

Govindassamy and Calemany and the same has been

duly registered in the Bahour Commune Panchayat.

My actual name is ‘Sinivassane’ and the same has

been found in my Birth Certificate, Family Ration

Card, Aadhaar Card, Electoral Identity Card, etc.

I have studied in the Government High School,

Karayambuthur. In my Transfer Certificate issued by

the said school authority; my name has been

mentioned as ‘Srinivasan’ instead of correctly

mentioning as ‘Sinivassane’. In the above matter

spelling mistake was caused by the said school

authority. I affirm that the above names ‘Sinivassane’

and ‘Srinivasan’ refer to one and the very same

person i.e., myself. I have been always using and

signing my name only as ‘Sinivassane’.
