I, Sonia Soni Singh, wife of Nemashanker Singh,

Hindu, a g e d about 37 years and residing at

37-A, First Floor, Goubert School Street, Vazhaikulam,

Puducherry, do hereby solemnly affirm and sincerely

state on oath as follows:

I am the deponent herein. I am well aware of the

facts of the affidavit.

I state that I married one Nemashanker on

23-11-2008 and begot one daughter ‘Deepti Singh

@ Hetvi Singh’ on 16-3-2011, which is the name

mentioned in her Birth Certificate, bearing

Registration No. P/2011/01833 issued by the Registrar

of Births and Deaths, Puducherry Municipality,

Puducherry. My daughter is called by name ‘Dia Singh’

as per the Certificate of Numerology.

I state that hereinafter she will only be called as

I further submit that my names ‘Amouda’, ‘Amuda’

and ‘Amudha’ denote one and the same person

i.e., myself. I further submit that I have also obtained

an Identification Certificate, dated 8-4-2013 issued

by the Hon’ble Principal District Munisif, Puducherry

in C.M.P. No. 68/2013, in which, it has been clearly
