I, B. Prabhakaran, son of Bakthavachalu, Hindu,

aged about 42 years, residing at No. 25, VI Cross

Street, Datchanamoorthy Nagar, Sokkanathanpet,

Puducherry-605 009, do hereby solemnly affirm and

sincerely state as follows:

I am an Indian national by birth. That a male child

was born to me and my wife Sarala on 31-7-2004

at JIPMER Hospital, Puducherry. The name of

the said male child who is our son ‘P. Sanjay’ as

mentioned (i) in his Birth Registration No. J/2004/6475,

dated 10-8-2004 made at Oulgaret Municipality;

(ii) in his Study Certificate, dated 15-4-2013 issued

by Principal, St. Patrick Matric Higher Secondary

School, Puducherry-605 005; (iii) in his Aadhaar

Card No. 3087 3847 9146 issued by Unique

Identification Authority of India and (iv) in the Family

Ration Card No. 114660, dated 15-7-2005.
