Puducherry, do hereby solemnly affirm and state as

1. That for and on behalf of myself and remitter
issue wholly renounce/relinquish and abandon the

Photo Identity Card (EPIC), bearing No. AWB0018614
issued by Electrol Registration Officer, Mahe
on 27-12-2010, in which my name is shown as
‘Chandrashekaran’.
5. I have changed my present name ‘G. Chandran’
as ‘Chandrashekaran’ for the reason that my
relatives, friends and native public popularly called
and known as ‘Chandrashekaran’ and for that purpose,
my name hereafter shall be called and known as
‘Chandrashekaran’ only.
6. That for the purpose of evidencing such my
determination declare that I shall at all times hereafter
in all records, deeds and writings and in all
proceedings, dealings and transactions private as
well as upon all occasions whatsoever use and sign
the name of ‘Chandrashekaran’ as my name/surname
in place and in substitution of my former name/
