3. Whereas in my Family Ration Card with number

179340 issued by the Department of Civil Supplies

and Consumer Affairs, Karaikal her name has been

4. To avoid confusion in my Family Ration Card

her name has to be altered as ‘ƒıxÔ ∏ˆBV’.

5. I undertake that I won’t seek any further

alterations in the Family Ration Card in the future.
