aged 47 years and residing at Old No. 80, New No.

158, Kosakadai Street (Ambalathadaiyar Madam

I state that I am the deponent herein and I am

Mark Sheet issued under the Authority of the

Government of Tamil Nadu and in my Bachelor of

Business Administration Degree Certificate issued

by Pondicherry University, Puducherry.

I state that my names ‘Kichorcoumar’, ‘Kishore

Kumar’ and ‘Kishor Kumar’ refer to one and the

same person i.e., myself only.

I state that my correct name and my name

henceforth be known as ‘Kichorcoumar’ only.

I further state that I made this affidavit in lieu of

proof of identity of my names.
