I, Haja Bahath, son of R.M. Aly and Habsa Ummal,

aged 32 years, Muslim, merchant, residing at No. 42,

Sunnambukara Street, Karaikal, do hereby solemnly

affirm and sincerely state as follows:

My birth name is ‘Haja Bahath’. In all the records

relating to me such as my Birth Certificate, Marriage

Certificate and in my Passport my name is given as ‘Haja

Bahath’. Now due to numerological factors, I have

decided to change my name as ‘Haja Fahad’. I hereby

give up and renounce my birth name ‘Haja Bahath’

and I shall henceforth be known and called only as
