CHANGE OF NAME

I, P. Karthiga, daughter of G. Pannir Selvam, Hindu,

aged about 20 years, residing at No. 96, Villianur Main

Road, G.P.K. Complex, Aziz Nagar, Reddiarpalayam,

Oulgaret Municipality, Puducherry-605 010, do hereby

solemnly affirm and declare as under :

I declare that I am a permanent resident of

Puducherry by birth and continuous residence. I was

born on 17-7-1993 and the said birth is duly registered.

My father’s name is ‘Pannir Selvam, son of Gurusamy’.

Previously, I used to put the initial for me as ‘P. Karthiga’.

Now, due to numerological reason, I am adding,

my grandfather’s name's first letter. Therefore, henceforth,

I shall be known only as ‘G.P. Karthiga’.

Solemnly and sincerely affirmed and signed before the
