I, D. Amaravathi, wife of (late) Durairaj, aged about

70 years, Indian, Hindu, widow and residing at No. 129,

North Street, Karukalacherry, Neravy Post, Karaikal

Town and District Munsifi, do hereby declare as follows:

1. That my last grandson is ‘Thulasidharan @

Manikandan’. He was born at Thiruvottiyur, Chennai

on 6-11-2003, to his parents Thiyagarajan and

Amutha @ Malathi. His birth was registered

with Thiruvottiyur Municipality with Registration

No. 685/2003/03, dated 7-11-2003. In his birth

register, his name is registered as ‘Thulasidharan’.

2. That ‘Thulasidharan @ Manikandan’ is a minor.

That I am his maternal grandmother i.e. mother of

Amutha @ Malathi. Right from his childhood, his

parents, myself, his elder sisters, friends and relatives

called him as ‘Manikandan’. So, ‘Manikandan’ was the

calling name of ‘Thulasidharan’. Hereinafter he will be

called as ‘Thulasidharan’.

Solemnly and sincerely affirmed and impressed my

left hand thumb-impression before the Notary Public

at Puducherry, on this 28th day of October 2013.
