I, Soultane Bivy Natchialle, daughter of Mohamed
Cassime Marecar and wife of Mohamed Saleem,
aged 41 years, Muslim, housewife, residing at No. 37,
Gnanapragasam Street, Karaikal, do hereby solemnly
affirm and sincerely state as follows:
I was born at Karaikal on 21-8-1972. My birth is
duly recorded in birth Registration No. K/1972/
01245 with Karaikal Municipality. In my said birth
register and my son Shafeek’s Birth Certificate my
name is correctly spelt as ‘Soultane Bivy
Natchialle’. In my daughter Hiyrun Sameema’s Birth
Certificate my name is entered as ‘Sulthan Beevi
Natchiyar’. Whereas in my Electoral Identity Card
No. KSQ0201913 my name is written as ‘Sultan
Natchial’. And whereas in my husband Mohamed
Saleem’s Indian Passport No. H 1929050 my name
is entered as ‘Sulthan Natchhar’.
I renounce and relinquish the names ‘Sulthan
Beevi Natchiyar’, ‘Sultan Natchial’ and ‘Sulthan
Natchhar’ and I shall hereafter be known, called,
referred and identified only as ‘Soultane Bivy
Natchialle’.

different spellings refer to me only and not any other
person.
