different spellings one is as ‘Mouralidasse’ the other

is as ‘J. Muralidass’ and the third is as ‘Muralidoss’.

I declare that the name with different spellings as

mentioned above are one and the same referring to

the same person, who is myself.

I further declare that the aforesaid name with three

different spellings refer to me only and not any other

person.

I further declare that in practice, I shall hereafter/

henceforth use my name with the spelling as

‘J. Mouralidasse’ for all purposes.

The above contents are true and correct to the

best of my knowledge and belief.

I state that I am the deponent herein and I am well

aware of the facts of this deposition.

I state that my name has been mentioned

as ‘Soucila’ in my Birth Certificate under

Registration No. P/1960/00031, dated 22-1-1960

issued by Puducherry Municipality, Puducherry and

in my Indian Passport No. G8005393 issued by

Regional Passport Office, Chennai.

I state that in my Voter’s Identity Card No. PY/01/

017/33258 issued by Election Commission of India my

name was entered correct spelling as ‘Suseela’ and

in my Family Ration Card No. 137646 issued by

Depatment of Civil Supplies and Consumer Affairs,

Puducherry my name was mentioned in Tamil

spelling as ‘∑EÈV’.

I further state that I am having only Election

Voter’s Identity Card as evidence for the documentary

proof in support of my correct name spelling.

I state that my names ‘Soucila’ and ‘Suseela’ refer

to one and the same person, i.e., myself only.

I state that my correct name spelling and my name

henceforth be known as ‘Suseela’ only.

I further state that I made this affidavit in lieu of

proof of identity of my names.

Hence this sworn affidavit may kindly be accepted

to this effect.

I take oath and solemnly declare/affirm that the

particulars furnished by me above are correct and

that I have not concealed or misrepresented

any facts.
