But in my Family Ration Card, my name has been

mistakenly mentioned as ‘Vijayakumaran’, which is

my home calling name.

Further, I declare that both the names ‘Munian’

and ‘Vijayakumaran’ are referring one person,

i.e., myself only.

I also declare that hereinafter I shall be known and

called only by the name of ‘Munian’.
