The other name is as ‘Z. Allaudeen @ Cheik Houssain’

as mentioned in my Marriage Contract Deed of my

marriage celebrated on 27-2-2003 with my wife Minhaj

Begam at Kottakuppam, Vanur Taluk, Tamil Nadu.

As such, I bear three names one is ‘Z. Allaudeen’,

the other is ‘Cheik Houssain’ and the third is as

‘Z. Allaudeen @ Cheik Houssain’.

That I have now changed my name from this

3rd day of February of the year 2014 onwards,

retaining the name as ‘Z. Allaudeen’ and abandoning

and relinquishing the other two names as ‘Cheik

Houssain’ and ‘Z. Allaudeen @ Cheik Houssain’ once

for all.

I would henceforth be known and called by my

retained name as ‘Z. Allaudeen’ for all purposes.

CHANGE OF NAME

I, R. Jeyabal @ Vijai Kshatriya, son of S.D. Rajasegaran,

Hindu, aged 29 years and resident of No. 12, Amman

Nagar, Katterikuppam, Puducherry-605 502, do hereby

solemnly affirm, declare and oath as under:

I declare that I am a permanent resident of

Puducherry by birth and continuous residence.

My name is entered as ‘Jeyabal’ in the Birth

Certificate, Passport, Degree Certificate and Family

Ration Card. I am also called as ‘Vijai Kshatriya’

which is my calling name. However, both the

aforesaid names find place in my Aadhaar Card.

Hence, I declare that both my names of viz., ‘Jeyabal’

and ‘ Vijai Kshatriya’ are refer to one and the same

person i.e. myself only. I further declare that

henceforth I shall be followed only the name of

‘Jeyabal @ Vijai Kshatriya’ in all my records.

This affidavit is made in lieu of proof of the

aforesaid facts and I have not concealed any

material facts.
