CHANGE OF NAME

I, Ragu @ Iyyanarappan, son of Ranganathan, aged

28 years, residing at Middle Street, Pudukuppam,

Ariankuppam, Puducherry-605 007, do hereby solemnly

declare and affirm on oath as follows:

I state that I am the deponent herein and I am well

aware of the facts of this deposition.

I state that my name has been mentioned as

‘Iyanarappan’ in my Birth Certificate under

Registration No. M/1984/00461, dated 23-1-1984

issued by the Registrar of Births and Deaths,

Puducherry Municipality, Puducherry and the same

name was mentioned as ‘Iyyanarappan’ in my School

Transfer Certificate issued by the Headmistress,

Perunthalaivar Kamaraj Government Boys’ High

School, Muthialpet, Puducherry and in my

Aadhaar Receipt No. 1190/09002/29227 issued

by Unique Identification Authority of India,

Government of India.

No. 392493 issued by the Department of Civil Supplies

and Consumer Affairs, Puducherry my name was entered

and was called by my relatives and others as ‘´z’.

I state that the names ‘Iyanarappan’ ‘Iyyanarappan’

and ‘´z’ refer to one and the same person, i.e. myself only.

I state that my correct name and my name

henceforth be known as ‘Iyyanarappan’ only.

I further state that I made this affidavit in lieu of

proof of identity of my names.

Hence this sworn affidavit may kindly be accepted

to this effect.

I take oath and solemnly declare/affirm that the

particulars furnished by me above are correct and that

I have not concealed or misrepresented any facts.

CORRIGENDUM

The following corrigendum is issued to point No. (27) of Annexure-II of the G.O.Ms. No. 29, dated 9-10-2013

of the Chief Secretariat (Education), Puducherry and published in the Extraordinary Gazette Part-II, No. 45,

dated 18-10-2013 in order to bring it on par with para 2(r) of Annexure-I in “The Guidelines for Implementation

of Students Special Bus Scheme”.

For

(27) This agreement is deemed to have come into

operation from the date of the signing of the agreement

and it is valid for a period of two years and it is also

terminable on three months notice on either side.

Read

(27) This agreement shall be in vogue till the expiry

of the existing permit which has been issued in the name

of the Director of School Education and it is also

terminable on three months notice on either side.

2. All other conditions remain unaltered.
