I state that my name has been incorrectly written

in my Family Ration Card as ‘Manimegalai’.

I do hereby make this affirmation for the proof of

my original and correct name as ‘Manimegala’ for

the purpose of getting entered the same in my

Family Ration Card in the place of wrongly entered

name ‘Manimegalai’ in order to avoid the

inconvenience in all affairs of my life in future.

I hereby state that then ‘Manimegala’ and

‘Manimegalai’ are referring one and the very same

person i.e. myself only.

The above deposition is true and correct to the

best of my knowledge and belief and nothing material

has been concealed therefrom.

‘Dinesh Sahar’ who was born on 26-8-2009.

2. I declare that my son was named as ‘Dinesh

Sahar’ at the time of his birth and is known so till

date.
