CHANGE OF NAME

I, K.P. Preetha, wife of Pavithran, aged 44 years, residing

at No. 2, A.G. Samy Street, Rajiv Nagar, Gorimedu,

Dhanvantri Nagar (Post), Puducherry, do hereby

solemnly and sincerely affirm and state on oath as

follows:

I submit that my name ‘Palli Kunnumel Preetha’

found in my Birth Certificate issued by the Registrar

of Births and Deaths, Yanam Municipality,

dated 21-9-1970 under Registration No. Y/1970/00106,

the name ‘K.P. Preetha’ found in my Transfer

Certificate, issued by the Headmistress, Government

Girls’ High School, Mahe, bearing Admission No. 3160,

the name ‘K.P. Preetha’ found in my Marriage

Certificate issued by the Commissioner, Mahe

Municipality, Mahe, the name ‘Preetha’ found in my

Livret de Famille issued by the Commissioner, Yanam

Municipality, the name ‘Preetha K.P.’ found in my

Bank Passbook issued by the Branch Manager,

State Bank of India, JIPMER bearing Account

No. 10100071950, the name ‘K.P. Preetha’ found in

my Aadhaar Card issued by the Unique Identification

Authority of India bearing No. 8388 9497 0364, the name

‘K.P. Preetha’ found in my Acknowledgment/Resident

Copy issued by the Unique Identification Authority

of India bearing No. 1190/09002/72437 and the name

‘Preetha’ found in my Family Ration Card issued by

the Department of Civil Supplies and Consumer

Affairs, Puducherry bearing Card No. 115859, refer

to one and the same person i.e. myself. I do affirm

that I shall hereafter be known only as ‘K.P. Preetha’.

That I hereby declare that I shall at all times

hereafter in all records, deeds and writings and in all

proceedings, dealings and transactions, private as

well as upon all occasions whatsoever use and sign

the name of ‘K.P. Preetha’ as my name in place and

in substitution of my former name.

The above deposition is true and correct to the

best of my knowledge and belief.

CHANGE OF NAME

I, Ambika, daughter of (late) Periassamy and wife of

Govindasamy, Hindu, aged 56, housewife, residing

at No. 29, II Main Road, Avvai Nagar, Lawspet,

Puducherry-605 008, do hereby solemnly and sincerely

affirm and state as follows:

I am the deponent in this affidavit and well

acquainted with the contents of the same.

I was born on 20-6-1957 at Bahour to my parents

Periassamy and Sinnaponnou and the same has been

duly registered in the Bahour Commune Panchayat

vide Registration No. 216. My actual name as per my

Birth Certificate is ‘Vedanayaguy’. I am also called as

‘Ambika’ and ‘Vedambika’. The name ‘Ambika’ finds

place in my Aadhaar Card bearing No. 6981 3271 1775

as well as in my Family Ration Card bearing

No.136268. The name ‘Vedambika’ finds place in my

Marriage Invitation. I affirm by this affidavit that the

aforesaid names ‘Ambika’, ‘Vedambika’ and

‘Vedanayaguy’ refer to one and the very same person,

i.e. myself only. But, I have been always using and

signing my name only as ‘Ambika’ for the past 35

years. Hereinafter I will be called and known only as

‘Ambika’.

CORRIGENDUM

In the Recruitment (Amendment) Rules, 2012 for the

post of Master Mechanic in the Agriculture Department,

Puducherry, notified in G. O. Ms. No. 21/Ag., dated

28-3-2013 of the Chief Secretariat (Agriculture and

Forests), Government of Puducherry, the Grade Pay

mentioned under Serial No. 4 of the Schedule, the

figures ` 2,800 shall be corrected and read as ` 2,400.
